The ElderCare Resource Center is an Alzheimer’s/dementia specific geriatric care management firm. We are highly specialized and experienced in working closely with memory impaired patients, their families, caregivers, and members of their health care team. Our goal is to keep our patient’s in the place they most want to be… their own home…
Utilizing our Integrated Care Management (ICM)® model, we coordinate, implement, and monitor all of the services necessary to improve the quality of care and the quality of life of the people we serve.
Although there may be no cure for Alzheimer’s disease, good planning and strong collaborative efforts by family members, caregivers, medical professionals, social management teams, and community organizations, can ease the burdens on the patient and their family.
The ElderCare Resource Center is conveniently located in West Palm Beach, Florida and has an additional office in Western North Carolina. In Florida, we serve the following areas: Boca Raton, Delray Beach, Boynton Beach, Lantana, Lake Worth, Wellington, Royal Palm Beach, West Palm Beach, Palm Beach, North Palm Beach, Singer Island, Jupiter, Palm Beach Gardens, Tequesta, Stuart & Pt. St Lucie. We also serve six (6) counties in South Florida including: Broward, Palm Beach, Martin, St. Lucie, Okeechobee and Indian River.
In Western North Carolina, we serve Canton, Clyde, Lake Junaluska, Maggie Valley, Asheville, Biltmore Forest, Black Mountain, Weaverville and all other cities located in Haywood and Buncombe Counties.Because we are highly sensitive to the range of emotions that accompany a new diagnosis and/or the progression of an existing illness, all clients are seen privately and by appointment only.
